About Us

A key part of the Campaspe Library Community Outreach Program, the Social Prescribing Library aims to link people to social and support activities within their community.
Working with Health Services we identify ways to encourage and enable individuals to take up opportunities to socialise and get involved in things that interest them.
What can be expected:
 Home visits - a purposeful social interaction in the individual’s home or place of residence.
 Signposting - to community based services that can improve health and wellbeing.
 Discovering - what is important for each individual’s wellbeing.
 Wellbeing sessions - programs and activities which support social, mental and emotional wellbeing.
 Support and encouragement to become involved in things that can help.

Target Audience:

The program is for people who need assistance with their mental health; have social needs which affect their wellbeing; feel lonely or isolated.

Eligibility Criteria:

Referrals are received from all health service partners including but not limited to Complex Care, Social Workers, Advanced Care, Hospital to Home, Transitional Care, and community health workers.
Families and community members are also able to refer family members, friends and neighbours.
