GP Social Work Connect

About Us

GP Social Work™ Connect is a free program offering short-term social work support to people in the Ryde, Hornsby and Ku-ring-gai Local Government Areas.

Our social workers are available to see you at your home or at your GP’s practice at a time that works for you. Working with you to set goals, they will assess your holistic needs and support you with a care plan that includes guidance, referrals and links to a range of relevant and appropriate local services, helping you overcome barriers.

Target Audience:

We can help people who
● have high or complex care needs that require support from a range of services
● have recent or recurrent hospital admissions
● have difficulty navigating and accessing support services, for example, Centrelink, the NDIS, Aged Care Services or Housing
● are experiencing disconnection, isolation or complex family situations
● are experiencing stress or fatigue from caring for a loved one.

Eligibility Criteria:

There’s no age limit to access this free service. We can help people in the Ryde, Hornsby and Ku-ring-gai Local Government Areas who
