The broader space of social prescribing recognises that much of wellbeing and quality of life sits beyond basic medical treatment. From loneliness to food insecurity to housing and quality of life, many of the interventions that make a difference to someone’s social, emotional and physical wellbeing can happen outside of the consultation room or hospital setting. Preventative health initiatives in communities play a significant role in enabling people to proactively manage their health and wellbeing before issues escalate.
Globally, there is greater recognition of how the social determinants of health (like education, employment, relationships, and living environments) impact health equity and healthy life expectancy. With increasing pressures on our health system there is also a growing need to connect people to resources in their communities. Social prescribing meets this need by linking individuals to local support and activities, benefiting their personal health and wellbeing, and strengthening community cohesion.
The Opportunity to Work Across Disciplines and Borders
EACH25 Conference, a trans-disciplinary conference, brings together leaders and practitioners in innovative social prescribing interventions. These are based on environment, daily activity, and meaningful connections with a common goal to improve health and wellness. They include art, heritage, culture, sport, leisure, nature, education and access to basic human essentials which contribute to better health outcomes and stronger communities.
The conference seeks to build an epistemic community that bridges disciplinary, geographic, and cultural boundaries. Together, we aim to exchange insights, forge strategic partnerships, and lead the way toward a future healthcare model where prevention, social wellbeing and community empowerment and are at the forefront of health policy and practice.


What does EACH stand for?
EACH is not an acronym, but a lens for understanding and designing total health:

Environment – the conditions in which we live, work, grow, and age—physical, social, cultural, and digital.

Activity – the daily routines, roles, and pursuits that give rhythm and meaning to our lives.

Connection – our relationships, sense of belonging, identity, and community.

Health – the full expression of wellbeing across mind, body, and life—shaped by both personal experience and structural conditions.
Social prescribing connects these building blocks and makes them part of healthcare—not as a referral pathway, but as a foundation for prevention and flourishing.
