About Us
The Social Prescribing trial program is a program aimed at support children and families who are or are at risk of social isolation and loneliness.
Working holistically with the client, or family, the Link Worker supports the clients in making decisions and exploring their individual, or family, options of what their ideal social connections may be. These can include light support, which provides a list of activities within the 'theme' of the clients wants, medium support, which goes into more detail, or supports a client to join an activity/event, and more high support which can be 'hand-held' support to take clients to the activities and support them during the activities/events.
Depending on the needs and the wants of the family/clients, the Link Worker will work with them to achieve the goals they set in order to make the necessary social connects they want.
The Link Worker can work with the family/clients until the client is satisfied to continue joining activities/events on their own.
