About Us
Get Connected is a community-designed and led social prescribing program that aims to reduce loneliness and isolation by fostering social connection in adults aged 25 years and older. Our Community Connectors support their peers to co-create a plan for social connection, together uncovering participants interests and passions, and where they can access them in the community.

Eligibility Criteria:
Participants must receive a referral from local social prescribing champions who have been onboarded to refer to Get Connected. Participants must be 25 years or older.
