The Community Connections Program
About Us
The Community Connections Program helps participants to enhance their wellbeing by linking them to local services and strengthening their community connections and support networks. ‘Community Connectors’ work with participants to assess their needs and identify meaningful goals. It’s a non-crisis, non-clinical, community-based program for up to 12 weeks, or more if needed. Information on how to access the program is available on the website, including an interactive map and brochures in multiple languages. The Program is funded by the Department of Human Services.

Target Audience:
The target population is people living in South Australia whose wellbeing is at heightened risk due to social isolation and/or loneliness and disconnection from vital supports.
Eligibility Criteria:
The program is for people aged 18-64 years and who are not receiving support from the NDIS or My Aged Care.
