Social Health Connect (SHC)

About Us

Social Health Connect (SHC) is a free, community-based social prescribing program delivered by Footprints Community for adults aged 18 years and over who have unmet social needs negatively impacting their health and wellbeing.
The program is delivered by skilled Link Workers, who work one-on-one with participants to identify personal goals, reduce social isolation, and strengthen social connection. Link Workers provide person-centred support, goal planning and follow-up, navigation of local community and social services, and practical assistance to overcome barriers to participation such as transport, confidence, language, or system complexity.
SHC takes a strengths-based, client-centred approach and supports participants to build capacity, confidence, and meaningful connections within their community, contributing to improved social, mental, and physical wellbeing

The program offers flexible and inclusive access options, including interpreter services upon request. Support can be delivered via phone, virtual, or community-based settings and is tailored to individual access needs, including cultural, language, mobility, and confidence considerations.

Target Audience:

The program supports adults who are experiencing loneliness, social isolation, or reduced connection to their community, as well as those living with social needs that impact their health and wellbeing. Participants may face barriers such as mental or physical health challenges, cultural or language barriers, transport issues, financial stress, or unstable housing. While the program is open to all eligible adults, it is particularly suited to individuals who would benefit from capacity-building, goal setting, and supported connections to community activities and services.

Eligibility Criteria:

Participants must be aged 18 years or older, be experiencing unmet social needs that negatively impact their health and wellbeing, and reside in the Caboolture, Redcliffe, or Kilcoy regions. Informed consent is required to participate. Referrals can be made by GPs or other health professionals, and individuals may also self-refer by contacting the service directly.
