A short term program supporting older Australians to access supports that assist in returning to pre-hospital life at home.

The program offers a mix of service and supports that can include assessment, planning, transport, information, care-coordination, referral services, as well as limited access to home assistance, welfare checks, and home visits where required.

Target Audience:

Older people in the process of being discharged from hospital and who meet one or more of the criteria.

Eligibility Criteria:

We can help people in the process of being discharged from hospital and who meet one or more of the following criteria:

They are living with three or more chronic health conditions/comorbidities
They are at risk of readmission to hospital after transitioning home
They would benefit from short term follow-up support
AND they are also residents of one of these local suburbs::

Hunters Hill
Lane Cove
Mosman
North Sydney
Northern Beaches
Ryde
Willoughby
*Clients may not be eligible if they receive a service package which already provides a comprehensive support service component, such as COMPAC, TRANSPAC, CHSP, HCP, NDIS etc.
