About Us

What is Social Prescribing? Social Prescribing is about supporting the health and wellbeing of people by using community based activities and supports, such as exercise, art, reading and gardening. Our Link Worker will talk to you about what activities interest you, the benefits of participating in them and will assist and support you in attending these activities. How it works: If you are eligible for this programme, you can be referred to our Link Worker by another service provider within our community or you can self refer by contacting us directly. You do not need a referral to access this free service. Our Link Worker will meet you to discuss your needs and the various Social Prescribing options available. You will begin your chosen activity with support Our Link Worker will meet with you after a number of sessions to discuss how you are enjoying your new activity and whether you would like to continue. They can make adjustments with you if needed to find the right fit!

Target Audience:

"The Program is for families within the Lockyer Valley with at least one child in your care under the age of 18 years. Parents/caregivers & children can all participate in this program. In long form..... The Lockyer Valley Social Prescribing Program can see the following;
o Adults of any marital status, in any kind of household composition, providing they have some form of caring responsibility for a child under 18 years. This includes foster and kinship carers.
o Adults of any marital status, in any kind of household composition, expecting a child (either through pregnancy, imminent adoption, or imminent foster or kinship care arrangement).
o Children (0-18), providing they are accompanied by a parent or guardian during the LWF service. (Social prescriptions may be made to a community activity that does not require parental/guardian supervision. This would be at the discretion of the parent/guardian.)
While our program will accept referrals for families with children in their care aged 0-18 years, we have chosen to place a special focus on supporting families with children aged 6-12 years. This decision reflects the recognition that our region already faces reduced access to services, and we aim to ensure inclusivity by not excluding families outside this age range. However, through extensive stakeholder discussions, it has become clear that children aged 6-12 years often exist in what many describe as ""limbo."" Many services are dedicated to supporting families with children under 6 years of age, such as early childhood programs, while others focus heavily on youth aged 13 and above. This leaves the middle childhood years underserved, with families reporting limited activities, opportunities, or supports tailored to this specific age group. By focusing on children aged 6-12 years, we seek to address this gap, providing targeted resources and connections that align with their developmental needs while also empowering their families with meaningful support."
