About Us

Our Social Prescribing Community Connections Program is part of Queensland’s Social Prescribing Trial, designed to reduce social isolation and loneliness among families with children by strengthening community engagement and wellbeing. Delivered through the Blackall Neighbourhood Centre, the program offers personalised support to help individuals and families connect with meaningful community activities, services and networks.

Our Team & Roles

Link Worker – Your First Point of Contact
Based at the Blackall-Tambo Community Hub, our Link Worker is the first point of contact for anyone seeking support through the program. This role involves:

Conducting holistic assessments to determine program eligibility.
Co-producing personalised plans with participants to identify goals and meaningful community connections.
Assessing and facilitating access to brokerage funding to overcome financial barriers to participation.
Mapping local community assets and services, including formal supports such as allied health, education, and specialist programs (e.g. learner driver schools).
Coordinating referrals and follow-ups to ensure participants are supported throughout their journey.

The Link Worker is not a case manager, but a connector and enabler—empowering families to build confidence, independence, and long-term wellbeing.

Community Connector – Walking Alongside You
Our Community Connector plays a vital role in supporting participants to engage with social activities in a gentle and inclusive way. This role includes:

Identifying and “gap-filling” community activities where opportunities for connection are limited.
Providing soft introductions to social settings, helping participants feel safe and supported as they engage in new experiences.
Assisting with transitions into community groups, events, or services, especially for those who may feel anxious or unsure.

This “walking alongside” approach ensures that participants are not just referred to activities, but are actively supported to engage in ways that feel comfortable and empowering.

Our Community Hub is wheelchair accessible. We offer flexible meeting locations, including community-based settings, where appropriate. We welcome families of all backgrounds, including Aboriginal and Torres Strait Islander peoples, CALD communities, LGBTQIA+ families, and those living with disability. We can provide translated materials and fact sheets in your preferred language. All our staff are trained in trauma-informed practice.

Target Audience:

Families with children, family members with shared caring responsibilities of children (grandparents)* requires assessment.

Eligibility Criteria:

Program is open to families with children who are experiencing, or are at risk of experiencing, social isolation or loneliness. This includes parents, carers, and guardians of children under 18, as well as expectant parents. The program is specifically designed to support families with children aged 0–18 years. However, the focus is on the whole family unit, and support is tailored to each family’s unique needs and goals. No formal referral is required. The program operates on a “no wrong door” approach, meaning individuals can self-refer or be referred by any community member, service provider, or organisation.
